• second form of ID (social security card, medical/car insurance, debt/cred card, etc.)

• 3 most recent pay stubs

• 2021 and 2020 W2s

• 2 most recent asset statements for all checking, savings, and investment accounts

Child support / alimony / separate maintenance income:

• divorce decree / court order (all pages)
• receipt of 12 most recent months of income 

 

Overtime or bonus income:

• last pay stub of 2021 and 2020

• 2021 and 2020 Federal tax returns (all pages, all schedules)

• 2021 and 2020 Business tax returns (all pages, all schedules)

• Current profit and loss statement
